Care Team

About The Role:

Provides companionship and practical support—conversation, social engagement, mail/organization, light housekeeping, meal planning/prep, errands, and non-medical transportation—delivered per agency standards and policies.

What You'll Do:

  • Offer friendship and emotional support; activities like conversation, games, reading; help clients stay connected.

  • Provide transportation for appointments/errands; accompany to social events; plan visits/outings.

  • Assist with correspondence and mail; teach/perform meal planning/prep; light housekeeping.

  • Participate on the care team; follow agency policies; monitor supplies; follow the care plan; assist in basic transfers only when client is independently ambulatory or with another trained helper present.

  • Reports to Supervisor; service delivered with respect and in line with agency standards.
